POLICY AND PROCEDURES

Thank you for choosing Dr. Jenkins as your healthcare specialist.  We are committed to your treatment being successful and your experience in our office being pleasant.  The following is an outline of our policies and procedures;

 

·        Office Hours- Monday – Thursday from 8:30am – 4:30pm and Friday from 8:30am – 12:00pm.  Closed for lunch daily from 12:00-1:00.

·        Medications will only be filled during office hours and are at the discretion of Dr. Jenkins.

·        Forms- A fee of $35.00 per form is required for the completion of forms, including but not limited to disability forms & FMLA. Please allow 7-10 days turnaround time for form completion.  Prepayment is required.

·        Medical Records-All medical record requests must be submitted in writing, a fee will be charged to release records with the exception of records released to another physician.  Please allow 7-10 days for records processing. 

·        Outpatient Scheduling-Please allow 5-7 business days for ancillary scheduling ordered by Dr. Jenkins. (E.g. MRI, Pain management, etc.) If your insurance carrier requires authorization it may delay scheduling.

·        Surgery Scheduling-Please allow 5-7 business days for surgery scheduling, once cleared.

·        Referral/Authorization-In the event your insurance requires a referral or authorization from your primary physician, it is YOUR responsibility to make arrangements with that office to get the referral/authorization to us prior to your appointment.  Your appointment will be rescheduled if the appropriate referral/authorization is not received.

·        Films-It is imperative to bring actual films, CD ROMS /DVD’S or films on disk.

·        Rescheduling-As Dr. Jenkins is a specialist surgeon, occasionally an emergency will necessitate the need to reschedule appointments.

·        Financial-In most cases we will bill your insurance for you. Please understand that this is a courtesy to our patients.  We do require you to pay your co-payment, co-insurance or deductibles at the time of service.  If you are unaware of what your benefits are, you should contact your benefits department prior to your appointment with Dr. Jenkins. It is your responsibility to understand the terms and benefits of your contract.  Any balance due from you after insurance has paid will be due within 30 days from receipt of your statement.  In the event a large balance is due, we can arrange for a payment plan.
